Dry Eye - OSDI test

The OSDI (Ocular Surface Disease Index®) Test is a diagnostic test that helps classify and measure the severity of dry eye syndrome based on its symptoms. Brudylab offers this anonymous online self-assessment tool. Alternatively, you can download the test to print it.

Please note that if symptoms of dry eye occur, consultation and examination by an ophthalmologist is recommended.

Download and print the OSDI Test in pdf.
Answer the following questions by selecting the box with the score that best defines the frequency of the symptoms.
Add up and write down the result for each block of questions (1, 2 and 3).
Add up the result and write it in the “Total” box.
Write down the total number of answers you have given (not counting those you marked "I don't know").
The formula for calculating the OSDI is “Total x 25 / Total number of responses”.
